Horizon 19 is a licensed Adult Day Program located in Antioch, California. We serve participants five days a week, for 5 hours a day. Our Person Centered Model empowers participants with the autonomy to choose their classes, such as Art, Health & Wellness, Life Skills, Adventures, Advocacy/Volunteers & Occupations, Explorers, and Music & Movement, offering participants opportunities to use and develop their learned skills.
We are accepting new clients. Our staff is committed to implementing a person-centered approach. We take pride in tailoring each client’s goals and interests and helping clients learn new skills and techniques. We offer scheduled activities designed to support growth, engagement, and independence.
